FORMS AND FEES 2021-2022
Parents can complete all the required forms for this school year inside of Final Forms. Final Forms can be accessed by using the “Quick Links” drop down menu on the District website.
- Only a parent signature is required on forms for students in grades K-6.
- A parent and student signature are required on forms for students in grades 7-12.
Students in grades 7-12 should log in to Final Forms using their @chagrinstudents.org email address. The password may be different from their usual password. If a student has forgotten their password, they can click the “Forgot Your Password” link on the student sign in page to request a password reset.
Student fees will be collected through Infinite Campus. Parents will need to sign into the Infinite Campus by using the “Quick Links” drop down menu on the district website. Once logged in, click on Fees to view and pay the fees associated with your student(s). You will need to add a payment method by clicking My Accounts, then Payment Methods, and clicking New at the bottom of the screen. A service charge of 4% will be added to all online payment transactions. Parents can choose to pay fees online through Infinite Campus or in person at your student(s) building.
